We understand that technology can occasionally be a hurdle. While we highly recommend using the onpatient portal for the fastest processing of your information, you may download and print our essential forms below if you are having difficulty accessing your account.
Please download, print, and complete the following documents prior to your appointment:
New Patient Intake Form: Comprehensive medical and psychiatric history.
Practice Consent Forms: HIPAA privacy policies, telehealth agreements, and office policies.
Release of Information(ROI): Only required if you would like us to coordinate care with your PCP or another provider.
To ensure your appointment begins on time, please return these forms at least 24 hours before your visit via one of the following methods:

In-Person: Drop them off at our Johns Island office located at 3227 Walter Dr, Ste B.
Important Note: If you choose to bring your forms with you to your appointment, please arrive 20 minutes early so our team can manually enter your information into your medical record before you meet with your provider.
Once you are logged into Onpatient, you can select your upcoming visit.
First Select Appointments from the tabs on top and then click on the date of the upcoming visit.
Then proceed to check in online. This can be done in advance of the appointment at any time!
Make sure you scroll to the bottom to complete the policies and procedures.
